The age of excess calories! The war against flesh, obesity


¡¼Korea Health Management Association¡½Jung-Hwan Park Professor, Department of Endocrinology and Metabolism, Hanyang University Hospital = We have recognized obesity simply as a risk factor for causing cosmetic problems or other diseases. However, as obesity patients and obesity-related diseases are rapidly increasing and related medical expenses are steadily increasing, obesity is considered as an independent disease.

 

In Korea, the number of obese patients is steadily increasing. One of these reasons is that we live in an age of overcalorie. It was about thirty years ago that I started living in abundance like this. Previously, we had to continue to live in poorly nutritious environments, so it was necessary to store more energy in our confined bodies.


Therefore, our body has evolved a way to store energy with energy-efficient fat, in a way to conserve fat for survival. Since these bodies do not change within a short period of time, we are easily exposed to a new disease called obesity these days, living in an age of excess calories.

 

¡Þ Waist circumference that measures only thin obesity

 

Some people simply think that when it comes to obesity, you usually weigh a lot. But to be precise, more fat has accumulated than normal. So even if you don't weigh a lot, you can be obese. Obesity is a state in which fat is accumulated more than normal, so measuring the amount of fat in the body is the most accurate way to diagnose obesity.


However, since it is difficult to accurately measure the actual fat mass, it is generally diagnosed through body mass index and waist measurement. Body mass index is the value obtained by dividing one's body weight (kg) by the square of height (§³). In Korea, if the body mass index is 25kg/§³ or more, it is diagnosed as obesity.


However, there is a problem in that the body mass index can be diagnosed with obesity even if there is not much body fat in case of a large amount of muscle such as an athlete. A way to compensate for this shortcoming of body mass index is waist measurement. Fat accumulated in our body includes subcutaneous fat and visceral fat. Visceral fat is known to have a greater effect on obesity-related diseases than subcutaneous fat.


¡°People who have only a skinny stomach,¡± often referred to as ¡°dry obesity,¡± may have a lot of visceral fat. Whether or not there is an excess of visceral fat can be seen through the waist circumference. The waist circumference is measured with a tape measure while exhaling comfortably.


As for the measurement position, measure the middle of the lowest position of the ribs and the highest position of the pelvis. When measuring, loosen the tape measure to the extent that it does not apply pressure to the soft tissue and measure it to 0.1cm. In Korea, when it is 90cm or more in adult males and 85cm in females, it is diagnosed as abdominal obesity with a lot of visceral fat.

 

¡Þ ¡°Low-tango diet¡± causes obesity?

 

Obesity is divided into primary obesity and secondary obesity. Primary obesity is caused by an increase in body fat when energy intake is greater than energy consumption, and secondary obesity is caused by inheritance, endocrine diseases, and drugs. Over 90% of obesity is primary obesity associated with calorie excess.


Primary obesity is caused by the interaction of genetic and environmental factors. Children can be obese in 80% if all parents are obese, 40% if one parent is obese, and 7% if all parents are not obese. However, two-thirds of obese patients were not obese when they were young, but it is known that they became obese when they became adults, and environmental factors are thought to be involved a lot.

 

The first of the environmental factors is the wrong diet type and habit, including overeating. With increasing interest in diet, more and more people are trying to ¡°low-tango diet¡±. In the case of low-tango diets, carbohydrate intake is reduced in the early stage, which is effective in weight loss, but ultimately, frequent intake of foods high in fat is the cause of obesity.


Recently there has been a craze for ¡°brown sugar,¡± but brown sugar is also a simple sugar like sugar, and excessive consumption can be a problem. Ingestion of beverages, snacks, and foods containing a lot of simple sugars is absorbed into the body faster than carbohydrates of polysaccharides such as grains, causing fat accumulation. In particular, excessive sugar intake is gradually addictive and tends to consume more sugar, which has recently been pointed out as an important cause of childhood obesity.

 

¡Þ Obesity increases the risk of death

 

Obesity does not stop with obesity, it can cause various diseases. Type 2 diabetes, dyslipidemia, high blood pressure, fatty liver, gallbladder disease, coronary artery disease such as angina pectoris or myocardial infarction, stroke, sleep apnea, gout, osteoarthritis, menstrual abnormalities, colon cancer, and breast cancer are typical obesity-related diseases.


In addition, obesity is known to increase the risk of death by 20%, and the risk of mortality increases as body mass index and waist circumference increase. The combination of diet control, exercise and behavior control is the most effective method in the treatment and prevention of obesity. Even if medication or surgical treatment is needed, management of lifestyle is essential.


During meal treatment, reduce energy intake while ingesting enough essential nutrients, and aim to lose weight to target. Although the weight loss effect of exercise is relatively small compared to other methods, it reduces the prevalence of obesity-related diseases and provides many additional health-related benefits.


Exercise 3 times a week high-intensity exercise, 200 minutes or more per week, or 2,500 kcal or more consuming aerobic and resistance exercise should be performed. Drug treatment cannot replace non-drug treatment such as diet or exercise for obesity, and should be performed as an aid while performing lifestyle correction.
