콜라겐의 효능, 거친 피부를 탱탱하게-노화 예방 기능

5년 전쯤 친한 친구인 피부과의사 친구가 저분자 콜라겐에 대하여 알려 달라했다. 그 당시만 해도 요즘처럼 콜라겐이 대유행을 하지 않을 때이다. 생화학을 전공한 나에게 콜라겐에 대하여 잘 알 것이라 생각하여 물었던 것인데, 사실 콜라겐은 단백질 중 가장 유용성이 떨어지는 단백질로 알고 있었을 뿐이다. 흔히 교원질로 불리는 콜라겐(collagen)은 대부분 동물 특히 포유동물에서 많이 발견이 되는 섬유성 단백질로, 피부와 연골 등 체내의 모든 결합조직의 대부분을 차지한다. 동물의 신체에 다양한 결합조직의 세포외 공간을 채워주는 주요 조직 형성 단백질이다. 포유동물에서는 전체 단백질의 25~35%를 차지하는 가장 풍부한 단백질이기도 하다. 콜라겐은 매우 긴 섬유상(fibril)의 형태로서, 힘줄이나 인대, 피부와 같은 섬유조직에서 흔히 볼 수 있다. 눈의 각막, 연골, 뼈, 혈관, 소화관, 척추 사이 원반, 치아의 상아질에서도 발견이 된다. 근육조직의 경우 근내막의 주요 구성 요소이다. 콜라겐은 근육조직의 1~2%를 차지하고, 강하고 힘줄이 많은 근육에서는 6% 정도를 차지한다. 음식과 의약품산업에서 흔히 사용되는 젤라틴(gelatin)은 콜라겐을 비가역적으로 가수분해한 것이다. 아이스크림, 젤리, 푸딩 등을 만드는 재료이고 약의 캡슐 원료로도 사용된다. 

 

 

콜라겐을 구성하는 아미노산은 매 3번째마다 글리신(glycin)이라는 아미노산이 존재한다. 다른 단백질에는 거의 들어 있지 않는 프롤린(prolin) 아미노산이 무려 17%나 들어 있을 정도로 다른 단백질과는 현저하게 다른 단백질이다. 프롤린의 경우 다른 아미노산과는 달리 닌하이드린 용액에 노란색을 띄는 유일한 아미노산으로 닌하이드린 반응에 노란색이 나오면 콜라겐일 가능성이 높다. 그러니 하나의 콜라겐은 약 1000개의 아미노산으로 구성되어 있어,  전체 아미노산 구성의 반 정도가 2가지 아미노산(글리신과 프롤린)으로 되어 있으며, 이들 아미노산은 8가지 필수 아미노산에 포함되지 않으며, 다른 구성 아미노산들도 거의 다 필수 아미노산이 아니어서 영양학적으로는 별 유용성이 없는 단백질이다. 즉 단백질이지만 질이 좋은 단백질이 아니다. 스트레스성 호르몬인 코르티솔은 피부의 콜라겐을 분해하여 아미노산으로 만드는데, 이런 이유로 스트레스가 피부 미용의 적이라 할 수 있다. 콜라겐이 피부의 탄력성을 유지하는데 작용하기 때문이다. 콜라겐은 분자량 10만의 폴리펩티드(아미노산과 아미노산이 결합한 것을 펩티드 결합이라 한다)가 나선 구조를 이루고, 이들 3가닥이 서로 묶여 초나선(superhelix) 구조를 이루어, 길이 300 nm, 직경 1.5 nm의 가늘고 길며 딱딱한 막대모양이다. 콜라겐이 나선구조를 형성할 수 있는 데는 매 3번째 마다 존재하는 글리신과 프롤린의 독특한 구조 때문이다. 이렇게 콜라겐이 나선구조를 이루는 데 있어 글리신이 중요한 이유는 초나선을 이룰 경우 안쪽의 공간이 없기 때문이다. 글리신은 곁사슬이 없는 유일한 아미노산으로 그 크기가 가장 작아 초나선 구조를 이루는데 있어 안쪽에딱 맞는 구조이다. 카르복시 말단 쪽에서부터 나선 구조를 이루기 시작하기 때문에 콜라겐의 998번째 아미노산이 글리신인데, 이 글리신이 곁사슬이 있는 아미노산으로 바뀌게 되면 초나선 구조를 이룰 수 없게 된다. 결과적으로 초나선 구조를 이루지 못한 콜라겐은 제대로 기능을 하지 못하게 되어 1,000여개 아미노산중 글리신 하나가 다른 아미노산으로 되는 돌연변이가 생긴 경우 태아가 출산 시 뼈가 제대로 형성되지 않아 출산 시 산도를 넓히지 못하고 오히려 뼈가 부러져 사산될 수 있다. 그만큼 콜라겐에서 글리신의 존재 이유가 크다. 프롤린은 아미노산이라 하지 않고 유일하게 이미노산이라 부르며, 그 구조가 천연적으로 구부러져 있어 콜라겐이 나선 구조를 이루는데 도움이 되는 아미노산이다.  콜라겐 분자의 양 끝은 이런 나선 구조를 이루지 않는 말단 펩티드라고 불리는 부분이 조금 있다. 즉 양 말단은 아미노산의 규칙성이 없어 매 3번째마다 글리신이 오지 않고 불규칙한 아미노산 배열을 가지기 때문이다. 콜라겐은 다른 단백질에 비해 항원성이 약하지만, 이 말단 펩티드가 항원성을 갖는다. 

  

콜라겐의 효능은 피부보습으로 콜라겐이 주로 피부 진피층에 주로 위치하고 있는 단백질로 수분을 끌어당기는 성질이 있어서 거친 피부를 촉촉하게 만들어주고 탱탱하고 탄력이 있는 피부로 만들어 주어 피부보습 기능을 갖는다. 또한 콜라겐은 노화 예방 기능이 있는데, 피부가 자외선에 오랜 시간 노출될 경우 단백질 분해효소가 증가되어 피부를 구성하는 콜라겐과 엘라스틴 섬유(탄력섬유) 등을 분해하여 피부 탄력이 떨어지게 되어 주름을 유발한다. 특히 25살 이후 콜라겐 생성 능력이 현저히 떨어지기 시작하므로 음식 섭취를 통해 보충하면 노화 예방에 도움이 된다. 나아가 콜라겐은 노인성 탈모예방 효과도 가지고 있다. 콜라겐이 피부를 재생하는데 중요한 역할을 하지만 우리 몸의 털을 만들 때도 꼭 필요한 성분으로 나이가 들면서 빠지거나 가늘어질 수 있는데, 평소 음식 섭취를 통해 콜라겐을 보충하면 노인성 탈모 예방 효과가 있다. 또한 콜라겐은 손톱 갈라짐을 예방한다. 손톱이 잘 부러지는 것은 단백질이 부족하기 때문인데, 단백질인 콜라겐을 보충하면 손톱에 윤기와 영양을 공급하여 단단하고 고르게 자라도록 도와주는 효과가 있어 손톱이 갈라지고 부러지는 것을 예방할 수 있다. 뿐만 아니라 콜라겐은 관절 건강에 도움이 된다. 관절은 뼈와 뼈가 연결되는 부위를 말하며, 움직임에 따라 발생하는 충격을 흡수하는 역할을 한다. 관절은 외부 충격이나 신체의 노화 등 여러 원인에 의해 연골에 손상이 발생하면서 통증이 나타날 수 있다. 뼈의 유기물 성분 중 73% 가량과 연골의 50% 가량이 콜라겐으로 구성되기 때문에 관절 건강에 도움이 된다. 콜라겐은 신진대사를 촉진한다. 콜라겐이 세포와 세포 사이를 연결해 주는 효과가 있기 때문에 소장에서 흡수된 영양소를 각 장기나 조직으로 운반시켜주는 역할을 하며, 신진대사를 촉진시키는 효과가 있어 몸에 축적된 각종 노폐물을 체외로 배출하는데 도움이 된다. 

 

콜라겐을 콜라겐 분해효소 이외의 펩신 등의 단백질 분해효소로 처리하면 나선구조에는 작용하지 않고 말단 펩티드에만 작용해서 말단 펩티드가 제거된 나선 구조의 콜라겐만 남는다. 이 콜라겐을 아텔로콜라겐(atelocollagen, a는 없다는 뜻이고 telo로 말단이니 말단 펩티드가 없는 나선구조만을 갖는 콜라겐)은 항원성이 거의 문제되지 않는다. 말단 펩티드는 분자와 분자 사이에 가교(crosslinking, 곁사슬 구조를 갖는 천연 또는 합성 고분자를 몇 가지 방법으로 연결하여 새로운 화학결합을 만들어 3차원의 그물망 구조를 하도록 하는 화학반응)를 형성함으로써, 생체 내에서 콜라겐은 섬유를 형성할 수 있으며 물에 녹지 않는 불용성이 된다. 이러한 아텔로콜라겐을 화장품에 이용하게 되는데, 아텔로콜라겐의 분산액(균일하게 분산된 액)을 동결 건조하여 스폰지 상태로 만들어 페이셜마스크 등에 이용하고 있다. 대부분의 경우 가용성 콜라겐을 크림, 유액, 화장수 등에 배합하여 사용하고 있다. 어린 동물의 피부 등에 존재하는 콜라겐은 아직 가교가 충분히 형성되지 않아 쉽게 산으로 추출되지만, 그 양이 적고 고가이어서 불용성 콜라겐을 가용화하여 이용하는 경우가 많다. 화장품에 콜라겐을 배합하면 보습성이 좋아지며, 특히 피부질감(texture)이 개선되어 사용감이 향상된다. 1% 콜라겐을 함유하는 누드마우스에 16일간 처리할 경우 표피의 두께가 증가되었는데, 콜라겐 분자량이 무려 30만이나 되어 침투하기 어려울 텐데 그 기전은 아직 확실하지가 않다. 

 

콜라겐은 pH 4 이상에서 침전되어, 화장품에 배합하기가 어려운데 콜라겐 분자의 곁사슬인 아미노기를 석신일화(succinylation)로 카르복실기로 변화시키면 등전점이 9.0에서 4.5로 낮아져 pH 3.5 이하 및 pH 5 이상에서도 가용성이 있어 배합하기가 쉬워진다. 음전하 계열의 화장품에 배합할 경우에도 콜라겐 곁사슬의 카르복실기를 에스테르화 하면 사용 pH에서 실효 전하를 +로 크게 바꾸어 보다 배합이 용이해진다. 콜라겐 가수산물(콜라겐 펩티드)도 화장품에 이용되고 있어, 콜라겐과 혼동되지만, 콜라겐 가수산물은 분자량도 훨씬 작어 수천으로 작고, 분자구조도 달라 그 효과가 다르다. 

 

20살이 넘으면 콜라겐이 서서히 체내에서 소실되기 시작한다. 콜라겐 수치는 처음에는 점차적으로 감소하지만 50살쯤 되면 폐경기와 더불어 가속화가 시작된다. 60~70살이 되면 생체 내에서 콜라겐 생성이 매우 낮아 대부분 사람들의 경우 눈에 띄게 노화 징후가 나타난다. 콜라겐은 피부뿐만 아니라 뼈, 연골, 장기, 혈관, 모발 등에도 분포하고 있어 지지대 역할을 한다. 노화가 진행됨에 따라 체내에서 콜라겐이 감소되기 때문에 구조적 장애가 발생할 수 있다. 

 

콜라겐의 체내 생산량은 연령이 높아짐에 따라 감소하는데, 생체 내에서 콜라겐이 생성되지 않기 때문에, 연령이 높아지면 식품 등으로 섭취를 할 필요가 있다. 콜라겐은 육류 및 어류에도 다량 존재하는데, 특히 돼지 껍데기, 닭 날개, 도가니, 닭발 등에 많이 존재한다고 알려져 있다. 그 외에도 곶감, 두충차, 감잎차 등의 식물성 식품에도 콜라겐이 존재한다.  콜라겐은 실제로는 콜라겐 가수산물인 아미노산이나 펩티드를 먹어서 몸에서 다시 콜라겐으로 이용되는 것이므로, 이용되기 쉬운 아미노산 또는 펩티드를 먹는 게 중요하다. 프롤린-하이드록시프롤린-글리신의 트리펩티드가 중요한데, 이 중에서도 프롤린과 하이드록시프롤린이 중요하다. 콜라겐 원료는 흔히 동물성으로 돼지 껍질이나 생선 비늘을 이용하며, 최근에는 식물성 콜라겐을 이용하기도 한다. 그러나 식물성 콜라겐은 함량이 매우 낮아서 콜라겐 공급에는 큰 의미 없다. 최근에는 어류에서 유래한 콜라겐이 주목을 받고 있다. 어류에서 유래한 콜라겐은 생선 비늘을 효소 분해하여 저분자 콜라겐을 만든다. 어류 콜라겐은 분자량이 500 달톤(Da, 바이러스 등 분자량 개념이 적합하지 않은 것의 질량단위) 정도로 분자량이 작아서 체내에 빠르게 흡수된다. 대신 대형 어류로 만들기 때문에 미세 플라스틱이나 중금속이 검출될 수 있는 위험이 있을 수 있다. 식용 어류로부터 얻은 콜라겐을 효소 처리하여 글리신, 프롤린, 하이드록시프롤린(Glycin-Proline-Hydroxyproline, GPH)과 같은 트리펩티드의 함량이 높은 콜라겐 가수산물을 얻을 수 있다. 이것이 저분자 콜라겐 가수산물(콜라겐 트리펩티드)이다. 비린내가 잔존할 수 있기 때문에, 생성 비늘 유래의 콜라겐을 식품 등에 함유하고자 하는 경우, 비린내를 가릴 수 있는 마스킹(차폐) 소재가 필요하다. 일례로, 대한민국공개특허 2013-0098729호에서는 녹차 추출물, 백차 추출물 등의 차 추출물을 이용하여 콜라겐의 이상한 맛이나 이상한 냄새를 감소시킬 수 있다고 알려져 있다. 하지만, 마스킹 소재와 같은 첨가물을 넣게 되면 첨가되는 마스킹 소재에 비례하여 콜라겐의 함량이 감소하는 문제가 발생한다. 음식물을 통한 콜라겐 흡수는 제한적이고, 효율성이 떨어지기 때문에 체내 흡수가 용이한 초저분자 수준의 어류 콜라겐을 섭취해 주는 것이 노약자들의 노화를 지연시키고, 건강한 삶을 살도록 하여 삶의 질이 높아진다. 그렇지만 생선에서 추출했기 때문에 중금속 위험을 고려하여야 한다.

 

식물성 콜라겐에는 동물성 콜라겐의 구성 성분인 하이드록시프롤린이 존재하지 않는다.  콜라겐을 섭취할 경우 체내에 작용하는 것이 수산화된 콜라겐이기 때문에 하이드록시프롤린은 콜라겐 작용에 있어 중요한데, 식물성 콜라겐에는 없다는 것이다. 1990년 아다이르(Steven Adair)와 메캄(Robert Mecham)이 저술한 식물과 동물의 세포외기질의 구조와 조합(Organization and assembly of plant and animal  extracellular matrix)에서 동물성 콜라겐에는 하이드록시프롤린이 풍부하지만 식물성에는 하이드록시프롤린이 발견되지 않는다고 명시하였다. 그러나 이후 식물성 콜라겐에도 일부 하이드록시프롤린이 있는 경우들이 보고되었다. 식물성 콜라겐은 히비스커스(Hibiscus, 하와이에서 훌라춤을 추는 사람들의 머리에 장식하는 꽃이며, 우리나라에서 금화규로 부르며 재배하기도 한다)와 카놀라 씨[개량된 유채, Canola는 Can(Canadian) + o(oil) + l(low) + a(acid)의 앞 글자를 따온 것이다. 국내에 들어온 카놀라는 거의 다 GMO(유전자 조작 식품)이다] 또는 버섯, 솔잎을 원료를 사용한다. 히비스커스의 경우 하이드록시프롤린이 13%나 들어 있다. 단점은 가격이 비싼 편이다. 카놀라 콜라겐은 비유전자 변이(non-GMO) 카놀라 씨에서 추출하며, 카놀라 씨를 저온 압착하여 콜라겐을 추출해 내면 파이토 콜라겐(phytocollagen)이 되는데 90% 이상이 분자량 100~400 달톤이고, 평균 500 달톤 이하로 가수분해 시킨 콜라겐이어서 흡수율이 90% 이상이다. 식물성 콜라겐에는 어류 콜라겐에 없는 폴리페놀이나 플라보노이드와 같은 항산화제가 풍부하게 들어 있어서 항산화 효과까지 기대할 수 있다. 석류 콜라겐과 타트체리 콜라겐도 많이 선전되고 있는데 석류 즙이나 타트체리에 어류 콜라겐을 섞은 것이다. 일종의 어류 콜라겐이라 할 수 있다. 

 

흔히 사용하는 콜라겐 보충제로는 콜라겐 가수산물(collagen hydrolysate)과 비변성 콜라겐(undenatured collagen type-II, UC-II, 콜라겐을 열이나 화학물질을 처리하지 않은 형태)이 있는데, 콜라겐 가수산물의 분자 크기가 작아서 흡수율이 높은 것으로 알려져 있다. 또한 골관절염의 통증감소에는 콜라겐 가수산물이 더 효과적이라는 연구가 있다. 골관절염의 강직감(관절운동 범위제한)에는 UC-II가 더 효과적이라는 연구 결과도 있다. 아마도 콜라겐 가수산물은 인대, 힘줄 등의 결합조직의 원료로 작용하고, UC-II는 일종의 면역 조절 작용이 존재한다고 추정한다. 

  

콜라겐 제품은 식약처로부터 피부 기능성을 인정받은 기능성 콜라겐과 기능성을 인정받지 못한 일반 식품으로서 일반 콜라겐으로 나눌 수 있다. 식약처로부터 기능성을 인정받으려면 여러 검증과정을 거쳐 기능성을 인정할 경우 건강기능식품으로 분류가 된다. 이에 관연 연구로는 2008년 펜실베이니아 주립대학의 클라크(Kristine Clark) 연구팀은 현재 의료 연구 및 의견(Current Medical Research and Opinion)에 발표한 논문에서 활동성 관절통이 있는 운동선수 147명(남자 72명, 여자 75명)을 무작위로 2그룹으로 나누어 식이보충제로 콜라겐 가수산물을 복용 후 24주간 관찰한 결과 콜라겐 가수산물 투여군에서 관절 악화 위험을 줄일 수 있음을 보고하였다. 2014년 독일 키엘대학의 프록쉬(Proksch E)가 피부 약리 생리학(Skin Pharmacol Physiol)에 발표한 논문에서 35~39세 여성 69명을 23명씩 3개 그룹으로 나누어 이중 맹검 위약 대조 실험을 진행한 결과 8주 동안 콜라겐을 섭취한 그룹만이 피부 탄력이 증가하였다. 2015년 시빌라(Sara Sibilla)가 의료 영양 및 뉴트라슈티칼 저널(J. of Medical Nutrition & Neutraceuticals) 논문에서 영국에서 45~64세 여성을 2 그룹으로 나눈 뒤 한 그룹에는 12주간 매일 콜라겐 보충제가 혼합된 음료를 제공하고, 대조그룹에는 위약 음료를 제공한 결과, 콜라겐 보충제를 섭취한 그룹은 9주간 주름이 점차적으로 감소하였으며, 12주간 피부의 수분함량이 대조군에 비해 높게 나타났다. 또한 자외선 조사에 의한 피부 표피 장벽 및 피부 탄력성 손상에 대한 콜라겐 가수분해산물이 미치는 영향을 분석한 실험에서 콜라겐 가수산물에 의해 경피 수분손실 증가와 각질층 수분농도 감소가 억제 되었으며, 피부 탄력 및 경피 히알루론산 수치가 개선되었다. 히알루론산은 피부 속에 수분을 저장했다가 보습이 필요한 곳에 전달하기 때문에 수분유지에 중요하다. 

   

콜라겐은 분말, 정제, 액상 등 다양한 형태가 있는데, 액상이 흡수가 빠르다. 그러나 먹는 콜라겐으로 만족스러운 효과를 보기란 사실상 어렵다. 콜라겐은 피부뿐만 아니라 관절, 손발톱, 머리카락 등 전신으로 흡수되기 때문이다. 얼굴 피부로 도달하는 양은 미미하다고 보면 된다. 인체에서 콜라겐을 구성요소로 하는 조직은 많다. 피부부터 시작하여 여러 인체 조직이 콜라겐으로 구성돼 있다. 그러다 보니 많은 사람이 콜라겐을 먹으면 마치 그 조직의 구성이 이루어지는 것으로 착각하고 있다. 하지만, 콜라겐을 먹는다고 그 조직이 만들어지는 것은 아니다. 단지 콜라겐도 결국 단백질 성분이므로, 그냥 영양제처럼 먹는다고 생각하면 된다. 중요한 것은 하이드록시프롤린이라는 아미노산이지만, 하이드록시프롤린이 단백질을 합성할 때 직접 들어가지 않는다. 단백질 합성 경우 일단 프롤린으로 들어가 수산화 과정을 거쳐 하이드록시프롤린이 된다. 광고하는 것처럼 직접적인 효과를 기대하기보다는 건강 기능식품을 섭취하는 것과 같다고 보는 것이 좋다. 

 

콜라겐 주사는 주사바늘을 사용해 짧은 시간에 안전한 주름제거 목적으로 사용된다. 효과 지속 시간이 반년에서 1년 반 정도로 그다지 길지 않지만, 흡수되어 부작용이나 합병증이 별로 없다. 인구 고령화 시대가 본격적으로 시작되면서 이에 따른 골관절염 환자도 꾸준히 증가하면서 관련 치료제 시장이 뜨거워지고 있다. 2021년 6월에 건강보험심사평가원은 관절강 주사제로 불리는 ‘슬관절강 내 주입용 치료재료’ 관련 급여기준을 6월부터 변경 시행 중이다. 즉 급여기준 상 기존 슬관절강 내 주입용 치료재료에 콜라겐 성분을 새롭게 추가하였다. 무릎 골관절염 환자 대상 기존 ‘폴리뉴클레오티드나트륨’만 존재하던 급여 항목에 ‘콜라겐’ 성분까지 추가 돼 2가지 성분의 관절강 주사제가 급여권에 포함되었다. 심평원은 콜라겐은 6개월 내 최대 5회 투여(1주에 1회씩, 총 콜라겐 투여용량 180 mg 이내)로 주사제 투여 방법 및 횟수를 명확히 했다. 발목을 삐어 정형외과에 가면 침도 맞고 물리치료도 받는데 효과가 그리 크게 없을 경우 프롤로 주사를 권하기도 한다. 프롤로는 주사제 이름이 아니라 프롤로테라피(prolotherapy) 즉 증식요법이라는 치료법이다. 증식치료란 약해진 인대나 힘줄, 섬유골 연결 부위 등에 증식제를 주입하여 그 주위에 젤라틴(콜라겐이 변성된 것)을 침착시켜 새로운 섬유조직 및 결합조직의 생성을 자극하여 그 부위를 강화시킬 목적으로 시행되는 시술이다. 프롤로 주사치료의 주성분은 포도당과 국소 마취제인 리도카인이다. 보통 수액으로 맞는 포도당보다 훨씬 고농도의 포도당이 사용된다. 바이오 콜라겐 조직재생 유도 주사 요법은 바이오 콜라겐 주사제를 인대와 힘줄 등의 손상 부위에 주사로 주입하는 치료법이다. 힘줄이나 인대 조직의 30~70%가 콜라겐으로 구성되며, 이들 조직이 재생되기 위해 콜라겐 공급이 도움을 줄 수 있으리라 기대한다. 조직에 따라 다르지만 30%~70%까지 공급하면 해당 조직의 재형성이 가능하다.

Graduated from Yonsei University College of Dentistry in 1980. 1988, Yonsei University, Department of Medicine, M.S.-Ph.D. 1987-1990, Professor, Department of Biochemistry, Wonju University, Yonsei University. 1990-1993, Visiting Professor, Department of Oncology, University of Wisconsin School of Medicine, USA. 1990-1996, Professor, Department of Biochemistry and Molecular Biology, Yonsei University College of Medicine. 1997-2018, Department of Oral Biology, College of Dentistry, Yonsei University. 2018- Present, Professor Emeritus, Yonsei University. He has served as the Director of Life Sciences and Medicine at the Other Academic Promotion Foundation and the Korea Research Foundation.
