중년 이후의 건강 위협하는 협심증

건강보험심사평가원 자료에 따르면 2020년 협심증으로 의료기관을 찾은 환자 수는 67만 4,598명으로 집계됐다. 2010년에 기록한 50만 3,825명보다 10만 명 이상 늘어난 수준이다.

 

발병 연령대로 보면 50세 이상 환자가 65만 4,666명으로 전체 환자의 97%를 차지했다. 그중에서도 50세 이상 남성 환자는 38만 4,672명으로 전체 환자의 절반 이상인 것으로 나타났다.

 

협심증으로 생기는 가슴통증의 특징

 

협심증 환자들은 ‘무거운 돌로 가슴을 누르는 것 같다’, ‘심장이 조이는 것 같다’고 표현한다.

 

가슴통증은 가만히 있을 때보다 운동 중일 때, 무거운 물건을 들었을 때 , 스트레스를 극심하게 받았을 때, 과식할 때 등 심장근육이 빨리 뛰거나 많은 일을 해야 할 때 잘 생긴다.

 

활동 시 온몸에 필요한 혈액을 공급하려면 심장이 더 빨리 뛰어야 하는데, 이때 좁아진 관상동맥으로 심장근육이 필요로 하는 산소 공급이 제한되면 증상이 나타난다. 운동을 멈추거나 스트레스가 없어지면 통증은 서서히 가라앉는다.

 

통증은 대개 1~15분 정도 지속되고, 간혹 어깨나 복부, 팔로 이어진다. 협심증 중 ‘변이형 협심증’은 동맥경화 때문에 발생하는 전형적인 협심증과 달리 관상동맥 경련이 원인이고, 주로 밤이나 새벽, 음주 후에 가슴통증이 생긴다.

 

가슴통증이 있다고 해서 무조건 협심증으로 오해하면 안 된다. 가슴통증을 유발하는 원인은 정말 다양하기 때문이다. 공복 시 속 쓰림 또는 식후 속 쓰림 등 식사 때문에 악화 또는 완화되는 가슴통증이 있는 경우에는 십이지장궤양이나 위궤양과 같은 질환을 먼저 의심해야 한다.

그 외에도 심리적 긴장, 불안에 따른 신체 증상으로서 가슴통증이 발생하기도 하는데 이 경우에는 운동을 포함한 신체 활동을 하면 오히려 증상이 완화되기도 한다.

 

증상만으로 원인을 완벽하게 구분하기 어렵지만, 협심증을 방치하면 심근경색 등 치명적인 질환으로 진행할 수 있다. 협심증 중에서도 ‘불안정형 협심증’은 심근경색으로 발전할 가능성이 크니 다음 세 가지를 꼭 기억하고, 한 가지라도 증상이 나타나면 즉시 병원을 찾아야 한다.

 

첫째, 4~6주 이내에 협심증으로 의심되는 가슴통증이 새롭게 발생했다. 둘째, 가슴통증이 나타날 때마다 강도가 증가하거나, 횟수가 점점 늘어난다. 셋째, 가슴통증이 안정 시에도 발생하거나, 휴식을 취해도 좋아지지 않는다.

 

동맥경화에 의한 관상동맥의 협착이 원인

 

협심증은 대부분 동맥경화에 의한 관상동맥의 협착이 원인이다. 동맥경화의 발생과 악화 원인으로는 고혈압, 흡연, 당뇨병, 비만, 고지혈증 등이 있으며, 그 외에 정신적 스트레스, 성급하고 경쟁적인 성격, 운동 부족 등을 들 수 있다.

 

동맥경화와 무관한 협심증의 원인으로는 관상동맥 경련, 대동맥판막 질환, 심한 심장비대 등이 있으며, 드물지만 심한 빈혈이나 갑상선항진증도 협심증의 원인이 될 수 있다. 중년 이후의 남성에게 많이 발생하고 안정형 협심증이 가장 흔하다.

 

안정형 협심증은 언덕길, 계단 등을 오르내리는 운동이나 심한 감정적 스트레스에 의해 유발된다. 증상 지속 시간은 길어야 10분, 대개 3~5분정도 계속된다.

 

대부분 안정을 취하면 통증이 바로 가라앉는다. 불안정협심증은 최근 새롭게 발생한 가슴통증, 강도와 빈도 측면에서 증상이 악화된 가슴통증, 안정 시에도 발생하는 가슴통증을 말한다. 이 불안정협심증은 심근경색으로 진전될 가능성이 크므로 특히 주의해야 한다.

앞가슴 한복판에 통증을 느끼고, 때로는 심장근육의 움직임이 떨어지는 등 심장근육의 산소 요구량과 공급량이 균형을 이루지 못할 때 발병하는 질환이다.

조기진단과 적절한 치료로 관리

 

당뇨병·고혈압·고지혈증 위험인자가 있거나 뇌졸중, 말초동맥질환 등과 같은 동맥경화와 관련된 질환을 이미 앓고 있으면 협심증이 생길 위험도 건강한 사람보다 높다. 협심증 가족력이 있는 사람도 마찬가지다. 고위험군이거나 가슴통증이 있다면 가볍게 여기지 말고 의사의 진료와 적절한 검사를 받아야 한다.

 

많은 협심증 환자가 병원에서 진단을 받은 뒤 “건강검진에서는 아무 문제가 없었는데 이상하다”라고 한다. 기본 건강검진으로는 협심증을 알수 없다.

 

기본 건강검진은 암 발견에 초점이 맞춰져 있어 심장을 자세히 알아볼 수 있는 검사 항목이 없다. 평소 심장 건강이 걱정된다면 관상동맥의 동맥경화 유무를 알기 위한 관상동맥 칼슘 CT 또는 관상동맥 CT를 추가해 검진을 받길 권한다.

 

협심증으로 진단받았다면 생활습관 개선이 첫 번째로 가장 중요하다. 동맥경화증 위험 요소를 조절하는 게 중요하므로 운동으로 체중조절, 식이조절 중요하며 그리고 금연은 절대적으로 필요하다. 두 번쨰로 고혈압, 당뇨, 이상지지혈등의 만선질환이 동반되었다면 이 질환의 약물 치료가 필수적이다.

 

세 번째로는 시술로 치료하는 방법에는 관상동맥 중재술이 있다. 소위 스텐트 시술로 불리는데, 이는 좁아진 관상동맥 부위를 풍선으로 확장한 뒤, 동맥이 다시 좁아지지 못하게 스텐트라는 그물망을 삽입하는 방법이다. 협심증 감소 효과가 좋아 많이 쓰인다. 단, 스텐스 시술을 받은 환자 중 10%는 1년 내 재발한다.

 

시술을 받았다고 해서 끝이 아니라, 생활습관 관리와 약물 복용을 꾸준히 병행해야 재발을 막을 수 있다. 마지막으로 좁아진 부위를 우회해 대동맥과 관상동맥을 이어주는 관상동맥우회술이라는 수술적 치료 방법이 있다. 수술적 치료는 관상동맥의 동맥경화 범위가 넓어 스텐트 시술이 쉽지 않은 환자에게 시행한다.

 

적절한 운동과 금연은 필수

환자들이 가장 어려워하는 것은 바로 금연이다. 제발 ‘금연하시라’고 해 도 지키지 못하는 환자가 많다.

 

스텐트 시술을 하고 몇 개월 괜찮아지니 한 개비 피우고, 그러다 ‘어 괜찮네’ 하면서 또 한 개비씩 피우다 금연 전 상태로 돌아가는 사람이 허다하다. 협심증이 있는데 담배를 계속 피우면 동맥경화가 계속 진행하고 결국 심근경색이 올 가능성이 커진다. 협심증이 있다면 금연은 필수다.

 

이 외에도 고혈압, 당뇨병, 고지혈증에 대한 적절한 치료, 체중조절 및 규칙적인 운동 등으로 위험인자에 대한 대책을 세워야 한다.

 

그리고 평소에 잡곡밥, 채소, 과일, 견과류, 콩류, 생선 등을 위주로 구성한 식사를 하고 붉은색 고기, 가공육류 등의 섭취는 줄여야 한다. 또 너무 과로하지 않도록 하고, 스트레스를 잘 해소하는 것도 중요하다.
